ABSTRACT

Objectives:To analyze the outcome of treatment of patients with multidrug-resistant tuberculosis (MDR-TB) in Shandong Province from 2013 to 2017, and to discuss the impact of the global fund's multi-drug prevention and control program (hereinafter referred to as “ the Program”) on the outcome of treatment of MDR-TB patients, so as to provide a basis for the formulation of MDR-TB prevention and control strategy in Shandong Province. Methods:Information of MDR-TB patients in Shandong Province from 2013 to 2017 was collected from the TB Management Information System, and Excel was used for statistical analysis. Descriptive analysis was conducted on the number of MDR-TB patients found, treatment rate, and successful treatment rate in the province, 5 project cities, and other non-project cities. SPSS16.0 statistical software was used for data analysis. Chi-square test was performed for comparison between rates (P<0.05). Result:The successful treatment rate in the province was the highest in 2014 (53.85%) and the lowest in 2017(22.93%), with a statistically significant difference (χ2=40.96,P<0.001). The treatment rate in the whole province increased year by year, from 66.97% to 85.06%, and the difference was statistically significant (χ2=30.53,P<0.001). The successful treatment rate was above 46% every year between 2013 and 2016, and decreased to 22.93% in 2017, with a statistically significant difference (χ2=40.96,P<0.001). The proportion of lost patients increased year by year since 2014, reaching 70.95% in 2017, with a statistically significant difference (χ2=2.32,P<0.001). In 2017, the number of patients found in the five project cities was the lowest in five years. Conclusion:The result of the project has an impact on the discovery, treatment and management of MDR-TB patients in Shandong Province. There is an urgent need to pay more attention to and actively explore prevention and treatment strategies for MDR-TB at the provincial level.

ABSTRACT

Infrasound widely exists in nature, our living condition, productive and traffic environment. Gastrointestinal tract is relatively sensitive to infrasound. However, the effect of infrasound on gastrointestinal function is unclear. Therefore, the purpose of our study was to observe the effects of infrasound on gastric motility and gastric morphology and to assess the expression of nitric oxide synthase (NOS) in gastric antrum after exposure to infrasound of 8 Hz - 130 dB for 2 hours per day for 14 consecutive days. Gastric motility was assessed by gastric fluid-emptying rate. Gastric morphology was evaluated by HE. The expression of NOS was measured by tissue microarray technology. The results would contribute to understand the role of infrasound in gastroenterology, and help to explain the mechanism of infrasound on gastroenterology.

ABSTRACT

<p><b>OBJECTIVE</b>To compare the expression of pinopodes, the marker of endometrial receptivity, during the implantation window in Kunming mice stimulated with two different doses of raloxifene (RAL).</p><p><b>METHODS</b>Forty-eight 8-week-old female Kunming mice were randomly divided into 4 groups (n=12), namely saline group, clomiphene citrate (CC, 18 mg/kg) group, RAL (33 mg/kg) group and RAL (44 mg/kg group). In each group, the mice received intragastric administration of 1 mL of normal saline containing CC or RAL at the specified doses or saline only as indicated for ovulation induction, once daily for 2 days. The mice received then injection with 5 IU human chorionic gonadotropin (HCG) and mated and on day 4.5 of gestation, the pregnant mice were sacrificed for examination of the uterus with scanning electron microscopy.</p><p><b>RESULTS</b>Abundant and well developed pinopodes were observed in the endometrium of the mice in the 2 RAL groups and in the saline control group. The mice in CC group showed obviously reduced endometrial pinopodes with poor development.</p><p><b>CONCLUSIONS</b>RAL at two different doses does not obviously affect the expression of pinopodes in the uterine epithelium of mice, suggesting the safety of RAL at these two doses for ovulation induction without causing adverse effects on endometrial receptivity.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare the diagnostic values of CD64 infection index, C-reactive protein (CRP), procalcitonin (PCT) and the percentage of neutrophils (NEU%) in leukemia patients complicated with bacterial infection.</p><p><b>METHODS</b>Sixty cases of leukemia complicated with bacterial infection (combination with infection group), 60 cases of leukemia without bacterial infection (non-combination with infection group) and 60 cases of healthy persons (control group) were selected in our hospital. CD64 infection index, CRP, PCT and NEU% were detected in the 3 group.</p><p><b>RESULTS</b>CD64 infection index, CRP, PCT and NEU% in combination with infection group were significantly higher than those in the non-combination with infection group and also significantly higher than those in the control group (all P<0.05). The CD64 infection index, CRP, PCT and NEU% in the non-combination with infection group were significantly higher than those in the control group (all P<0.05). The sensitivities of CD64, CRP, PCT and NEU% in the diagnosis of the leukemia with bacterial infection were 71.06%, 84.86%, 66.93% and 59.25% respectively, and the specificities of CD64, CRP, PCT and NEU% were 91.46%, 75.94%, 88.79% and 85.36% respectively.</p><p><b>CONCLUSION</b>Compared with CRP, PCT and NEU%, CD64 infection index for diagnosis of leukemia complicated with bacterial infection has higher specificity, which is helpful for the early diagnosis of the disease.</p>

ABSTRACT

This study aimed to learn the recurrence rate in the retreatment TB patients with sputum smear and/or culture positive (ss+ and/or c+) two years after they were declared cured, and to explore causes of recurrence in order to improve long-time treatment outcome. 5 cities were selected as research locations. Recurrence of TB was judged by chest X-ray examination together with sputum smear and culture examination. Questionnaire was carried out to collect data on treatment history, demographics and possible risk factors. Chi-square test and logistic regression were conducted using SPSS in this study. As the result, 99 active TB patients were identified and the recurrence rate was 16.67% and irregular medicine and smoking were the causes of the recurrence. The retreatment TB patients with sputum smear and/or culture positive in this study had higher recurrence rate and health education work on regular medication and smoking should be strengthened.

ABSTRACT

<p><b>BACKGROUND</b>Genetic variations at the interleukin 28B (IL-28B) locus are important in predicting outcome following therapy for chronic hepatitis C virus (HCV) infection. The aim of this research was to evaluate the role of IL-28B single nucleotide polymorphism (SNP) variations in Chinese patients undergoing pegylated interferon-α plus ribavirin (PEG-IFN-α/RBV) treatment.</p><p><b>METHODS</b>To determine the effect of IL-28B variation on the response to HCV therapy, these variants were genotyped in a cohort of 220 patients who were chronically infected with HCV and received combined PEG-IFN-α/RBV therapy.</p><p><b>RESULTS</b>The proportions of rs12979860 CC, CT, and TT genotypes were 71.4%, 25.0%, and 3.6% respectively, in the sustained virological response (SVR) group; 15.8%, 60.5%, and 23.7% respectively, in the null virological response (NVR) group; and 38.1%, 52.4%, and 9.5% respectively, in the relapse (Rel) group (P < 0.05). Logistic regression analysis showed that, compared to those having the CC genotype, CT heterozygotes had an increased risk of NVR and Rel (OR = 10.95, 95%CI = 4.12-29.11, P = 1.5×10(-7) and OR = 3.93, 95%CI = 1.86-8.32, P = 2.1×10(-4) respectively). The RNA quantification assay showed that patients with genotype CC exhibited much higher levels of IL-28 expression than those with genotype CT or TT (P < 0.001).</p><p><b>CONCLUSIONS</b>The IL-28B SNP rs12979860 genotype was related to the effectiveness of HCV therapy: patients with the CC rs12979860 genotype had higher rates of SVR than those with the CT or TT genotype, and the CC genotype revealed a significantly higher level of IL-28 mRNA expression.</p>

Fragile X mental retardation 1 is the gene underlying fragile X syndrome (FXS). Its product, fragile X mental retardation protein, is closely involved with development of brain and neurons. PCR and Southern blotting have been the major methods for laboratory diagnosis of FXS. In this article, the progress in the molecular diagnosis of FXS is reviewed.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the relationship between polymorphism in the interleukin (IL)-28B gene and sustained virologic response (SVR) in chronic hepatitis C (CHC) patients.</p><p><b>METHODS</b>A total of 220 patients with CHC were prospectively treated with pegylated-interferon (peg-IFN) in combination with ribavirin (RBV) for 48 weeks, and followed-up for an additional 24 weeks. All patients were genotyped for the rs8099917 polymorphism and correlations with antiviral efficacy were determined by statistical analysis.</p><p><b>RESULTS</b>One-hundred-and-eighty-two (82.7%) of the patients achieved end-of-treatment virological response (ETVR). Significantly more patients in the ETVR group carried the rs8099917 genotypes of TT (93.5%) and GT+GG (68.8%), compared to the patients who did not achieve ETVR (X2=23.287, P less than 0.01). In addition, the patients who achieved SVR also represented significantly higher rates of both genotypes (TT: 86.2% and GT+GG: 60.6%; X2=15.531, P less than 0.01). In the SVR group: TT vs. GT+GG: odds ratio (OR)=4.063, 95% confidence interval (CI): 1.972-8.369; X2=15.531, P less than 0.01. In the RP group: TT vs. GT+GG: OR=0.246, 95% CI: 0.119-0.507; X2=15.531, P less than 0.01).</p><p><b>CONCLUSION</b>The IL-28B rs8099917 genotype is closely related to antiviral response of patients with chronic hepatitis C. Compared to carriers of the GT and GG genotypes, carriers of the TT genotype have higher SVR rates and lower RP rates. The TT genotype may be an important predictor of antiviral efficacy.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To detect NF1 gene mutation in a patient with neurofibromatosis type 1.</p><p><b>METHODS</b>Five fragments encompassing the entire coding sequence of the NF1 gene were amplified with reverse transcription PCR. PCR products were directly sequenced. Suspected mutations were verified by sequencing of DNA amplified by PCR using genomic DNA as template. Corresponding exon of family members was also sequenced. Furthermore, the PCR products were inserted into a pGEM-T cloning vector to quantify cells carrying the mutation in different samples derived from the three embryonic layers.</p><p><b>RESULTS</b>The proband's clinical manifestation was consistent with neurofibromatosis type 1. Sequence analysis has identified a novel heterozygous mutation c.7911 C to T (p.Q2510X) in exon 51 of the NF1 gene in the proband. The same mutation was also detected in peripheral blood cells, uroepithelial cells and oral mucosal cells of the proband, though the signals of uroepithelial cells were significantly weaker. By T cloning-sequencing, recombinants carrying the NF1 gene mutation respectively accounted for 42%, 36% and 12% of all peripheral blood cells, oral mucosal cells and uroepithelial cells .</p><p><b>CONCLUSION</b>It is likely that a mutation of NF1 gene has occurred in early embryogenesis of the proband, which in turn has led to generalized mosaicism of neurofibromatosis type 1.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the clinical value of examination of cervical HPV infection in women in early pregnancy and postpartum.</p><p><b>METHODS</b>Using flow-through hybridization and gene chip techniques, we examined 3 806 cervical specimens of pregnant and postpartum women of different ages with different cervical diseases. The women were grouped into different age groups by every 5 years for HPV DNA genotyping of the specimens, with another 4080 women without pregnancy serving as the control.</p><p><b>RESULTS</b>Of the total of 7886 specimens, high-risk HPV infection was detected a the rate of 12.5%. In pregnancy, postpartum and nonpregnancy, the infection rate was 14.3%,, 10.5%, and 11.7%, respectively. In the 4 age groups, the infection rate was 16.9%, 12.1%, 13.8%, and 22.1%, respectively.</p><p><b>CONCLUSION</b>The high-risk HPV infection rate in pregnancy differs significantly from that in nonpregnancy and postpartum. The infection rate also differs with age during pregnancy and postpartum. Examination of HPV infection during pregnancy is safe and feasible.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the expression of c-myb in reflux esophagitis, Barrett esophagus and esophageal adenocarcinoma.</p><p><b>METHODS</b>The expression levels of c-myb in the esophageal mucosa tissue of patients with reflux esophagitis, Barrett esophagus and esophageal adenocarcinoma were detected by real-time fluorescent quantitative RT-PCR.</p><p><b>RESULTS</b>The expression levels of c-myb mRNA in Barrett esophagus and esophageal adenocarcinoma tissues was significantly higher than that in normal and reflux esophagitis esophageal tissue (P<0.05 or 0.01), and increased progressively with the development of reflux esophagitis into Barrett esophagus and esophageal adenocarcinoma.</p><p><b>CONCLUSIONS</b>The expression level of c-myb mRNA is closely related with the development of Barrett esophagus and esophageal adenocarcinoma, and may be used as a valuable index for monitoring the early onset and intervention of Barrett esophagus and esophageal adenocarcinoma.</p>

ABSTRACT

Objective: To investigate the effects of peptide YY (PYY) on the interdigestive migrating myoelectric complex (MMC) in the small intestine in vivo and explore the neural and endocrinal mechanisms of the effects. Methods: Sprague-Dawley rats were supplied with a venous catheter and bipolar electrodes in the duodenum and jejunum for electromyography of stomach and small intestine in wake state. PYY, phentolamine, nitro-L-arginine (L-NNA, the inhibitor of nitric oxide synthase) and atropine were served with PYY respectively. The plasma motilin levels before and after the infusion of PYY were observed. Results: At all the three recording points, PYY lengthened the circle length of MMC [from (591.90±128.98)s to (999.25±216.59)s, P<0.01] and lowered the frequency of phase III [from (39.28±8.40) min-1 to (22.08±3.13) min-1, P<0.01], amplitude of phase III [from (0.320±0.060)mV to (0.179±0.030)mV, P<0.01], and the portion of phase III over the whole circle length [from (28.61±5.84)% to (15.43±5.16)%, P<0.01]. Phentolamine had no influence on the role of PYY. Administered L-NNA combined with PYY, the percentage of phase III increased [(42.09±8.67)%] compared with that of control (P<0.01) and compared with that of PYY administered alone (P<0.01) too. Atropine combined with PYY showed stronger depressing effects on MMC. No significant difference was found between the plasma motilin levels before and after the infusion of PYY. Conclusion PYY may inhibit the interdigestive intestine motility through the none-adrenergic none-cholinergic tract, while the α-receptor tract and circulating motilin are probably not involved in the depressing effect.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the effect of the implementation of CIDA/WHO TB Control project in Shangdong Province.</p><p><b>METHODS</b>The statistical data and supervision information of 42 counties involved in the project from September 2002 to December 2005 were collected and analyzed.</p><p><b>RESULTS</b>The recorded rate of smear-positive TB patients increased from 23.89 per 100 000 persons to 41.04 per 100 000 persons after 3 turns of project in the 3 years. The cure rate in initially smear-positive patients was 97.21% among 14 592 patients having completed the course of treatment; it was 95.36% among 4978 recurred smear-positive patients after the treatment. Thus, the goal aiming at high rate of diagnosing and curing was achieved.</p><p><b>CONCLUSION</b>The successful implementation of the project has improved the diagnose and treatment of infectious pulmonary TB patients in Shangdong Province and facilitate the implementation of Provincial TB Control Program.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the risk factors of Schistosoma japonicum reinfection after chemotherapy in a hilly endemic area.</p><p><b>METHODS</b>In a hilly endemic area of S. japonica, data on contact to contaminated water and the rates of reinfection after mass praziquantel treatment were collected from residents who participated in a prospective cohort study on S. japonica. An unconditional logistic regression analysis was performed to identify the risk factors associated with the reinfection of S. japonicum.</p><p><b>RESULTS</b>Logistic regression analysis data showed that three factors were associated with S. japonicum reinfection: eggs per gram stool (EPG) prior to chemotherapy (OR = 2.066, 95% CI: 1.173 - 3.639), gender (OR = 4.260, 95% CI: 1.275 - 14.235) and average degree of exposure (Index B) from April to October (OR = 1.138, 95% CI: 1.045 - 1.240). Interaction between gender and average exposure Index B from April to October was identified (OR = 0.875, 95% CI: 0.817 - 0.982).</p><p><b>CONCLUSION</b>Eggs per gram stool (EPG) prior to chemotherapy, level of exposure to infected water and gender were risk factors for reinfection with S. japonicum after chemotherapy. In addition, there appeared an antagonism between gender and level of exposure to infected water.</p>

ABSTRACT

Objective To study the effects of infrasound of different frequencies and intensities on the levels of somatostatin(SS)and cholecystokinin(CCK)in rat duodenal mucosa.Methods A total of 140 male Sprague- Dawley rats were randomly divided into a control group,an 8 Hz-90 dB group,a 8 Hz-130 dB group and a 16 Hz- 130 dB group.The experimental groups were exposed to infrasound of 8 Hz-90 dB,8 Hz-130 dB or 16 Hz-130 dB,re- spectively,for 2 hours a day,and the control group was put under sham infrasonic exposure.After being exposed for 1, 7,14,21 and 28 days,7 rats were randomly selected from each group and underwent SS and CCK levels detection in duodenal mucosa by microarray radioimmunity method.Results Compared with the control group,the SS and CCK levels of duodenal mucosa did not change significantly after exposure for 1 day.The SS and CCK levels of duodenal mu- cosa significantly increased in 8 Hz-90 dB group,8 Hz-130 dB group and 16 Hz-130 dB group after exposure for 7,14, 21 and 28 days(P

ABSTRACT

Objective To observe the effects of infrasound(IS)of different frequencies and intensities on the pathological morphology of the gastric mucosa(GM)in rats.Methods One hundred and forty male Sprague- Dawley rats were randomly and evenly divided into a control group(group A),an 8 Hz 90 dB group(group B),an 8 Hz 130 dB group(group C)and a 16 Hz 130 dB group(group D).IS with these frequencies and intensities was administered daily for 2 h to all groups except group A,which received sham infrasound.The other 70 rats were ran- domly and evenly divided into a second control group(group E)and an exposure group(group F),in which the rats were continuously stimulated by IS at 8 Hz and 130 dB for 2 h a day for 14 d.The pathological morphology of the GM in each group was observed at 1 d,7 d,14 d,21 d and 28 d after IS exposure.Results①Compared with group A,GM lesion scores were significantly increased in groups B,C and D at 1 d,7 d,14 d,21 d and 28 d(P<0.01),but not in group B at 1 d.②Compared with group B,the GM lesion scores in group C were obviously in- creased at 1 d,7 d,14 d,21 d and 28 d,while scores were also obviously improved in group C in comparison with those in group D at 14 d,21 d and 28 d(P<0.01).③The GM lesion scores in group F decreased gradually after IS,but were still higher than those in group E at 1 d,7 d,14 d,21 d and 28 d after IS.④The ultrastructures of the chondrosome and endocytoplasmic reticulum in GM cells were deformed after 8 Hz and 16 Hz IS.Conclusion 8 Hz 90 dB,8 Hz 130 dB and 16 Hz 130 dB IS can all result in GM damage in rats.The injury severity was closely related to the frequency,intensity and duration of the IS.Rats can adapt to IS after several exposures,and the damage tends to recover automatically.

Objective To analyze the incidence rate and mortality of perinatal asphyxia and effects of new resuscitation technique on asphyxia as well as the risk factors of asphyxia in latest 10 years.Methods A retrospective evaluation was done for all the newborns who were born in the provincial women and children′s health care hospital from 1995 to 2004.The morbidity,mortality and fatality rate were calculated for each observed year and different seasons.The influence of gender,body weight,gestational age as well as polyembryony and mode of delivery on the asphyxia was analyzed.Results The morbidity of mild birth asphyxia was decreased dramatically and maintained at about 1.5% after using new resuscitation technique,however,there were no obvious effects on the sever asphyxia.In the same time,no big influence on fatality rate of birth asphyxia was observed.The incidence rate was highest in April,but the mortality and fatality of asphyxia was highest in July.The incidence of asphyxia was also related with gender,polyembryony,birth weight,prematurity babies and aids to delivery from voginal.Conclusions The incidence of perinatal asphyxia is related with the gender,polyembryony,birth weight and gestation age as well as seasons.New resuscitation technique can reduce the morbidity of mild birth asphyxia,and no effect on the severe asphyxia as well as fatality rate.

ABSTRACT

Objective To explore the effect of systemic subhypothermia and erythropoietin combined treatment on neonatal hypoxic-ischemic brain damage(HIBD).Methods Sixty-five asphyxiated newborns enrolled in this study were divided randomly into combination group(n=16),hypothermia group(n=24)and conventional group(n=25).Subhypothermia was adepted in hypotheria group as well as conventional therapy.Subhypotheria and erythropoietin were both applied in combination group excepy for conventional therapy.The short-term effect was evaluated by neonatal neurological score as well as neonatal behavioral neurological assessment(NBNA)on postnatal days of 7,14 and 28,and the long-term effect was evaluated by using intellectual development table made by Children's Development Center of China(CDCC)at 3 and 6 month of age.Results The neonatal neurological score in combination group at 24,48,72 and 80 h were lower than that of 0 and 12 h.NBNA evaluation was much higher on 14 d and 28 d in combination group and hypothermia group than that in conventional group(Pa0.05).Conclusions Combination therapy with systemic subhypothermia and erythropoietin exerts obvious short and longer-time neuroprotective effect on HIBD,but there are no differences compared with hypothermia alone.
